110-300-0165(2)(c) Licensee has several extension cords around the outdoor and indoor play areas that are accessible to the children but licensee stated she will be removing all extension cords as soon as possible. Licensee had two black hoses laying outside that can cause a loop around a child’s neck. 110-300-0165(2)(d) Licensee had a large ripped black plastic bag accessible outside. 110-300-0165(3)(a) Licensee had two large wood apple bins that are broken and splintering, plastic yellow stool, ripped tarp, 12 inches metal stakes in garden area, wood climber wall has a warped board that needs to be replaced and refinished due to splintered edges. 110-300-0165(d) Licensee had washer, dryer and second freezer unlocked and accessible, in the kitchen area while on this site visit.
